Appointments and Cancellations We see patients by appointment. You can complete Patient Forms prior to your office visit. Simply fill out and print and bring them with you, or fax to 248.661.6210.
Please call our office during regular business hours to schedule your appointment. We will make every effort to accommodate emergency appointments within 24 hours.
Need to speak to your doctor? Please call during business hours. A message will be taken and every attempt will be made to return your call by the end of the day.
Prescriptions Prescription refills within 48 hours. Please have your pharmacy phone number available.
New Patients New patients should arrive 15 minutes prior to your scheduled appointment.
Please bring: -your insurance card -your driver's license -any relevant X-Rays and other test results -a physician referral -a list of your current medications -Patient Forms
Payment and Insurance Policies Co-pay is due at the time of service. If you do not have insurance, payment is expected in full at the time of service, unless alternate payment arrangements are made prior to your appointment. We accept cash, checks, and credit cards.
We participate with many insurance companies, including Medicare and Blue Cross/Blue Shield. We will file all insurance claims with the information that you provide to us. We remind you that insurance coverage is an agreement between you and the insurance carrier. While we will assist you in every way possible, the financial responsibility for any unpaid balance will be yours. Please check with your insurance company prior to your appointment and with our office for the most current list of insurance plans we participate with.
